Are you a Registered Children's Nurse ready to step into a clinical leadership role? We're looking for a Children's Nurse Case Manager to join Routes Healthcare's specialist complex care team. You'll take clinical ownership of a caseload of children with complex needs — leading assessments, overseeing care delivery, and supporting frontline staff.

What the role involves:
• Clinical case management of children with complex care needs
• Conducting and reviewing risk assessments, care plans, and clinical documentation
• Providing clinical supervision and competency sign-off for care staff
• Acting as the named clinical lead for your caseload
• Supporting new package set-up, including initial assessments and staff induction
What we're looking for:
• Registered Children's Nurse (RCN) with a valid NMC pin
• Proven experience in tracheostomy and ventilation care — essential
• Previous case management, community nursing, or clinical leadership experience — highly desirable
• Confident communicator with families, clinical teams, and commissioners
